Population Management (panel management)

Primary care clinicians are responsible for the preventative, acute and chronic healthcare needs of a cohort or “panel” of patients. Whereas acute care needs are often best addressed in a face-to-face office visit, chronic disease and preventative care can be more effectively managed with alternative, team-based approaches. Panel management is a method by which we ensure that all of a clinician’s patients are receiving appropriate chronic disease care and preventative measures regardless of whether they have been seen recently for an office visit.
